COMPREHENSIVE HOSPICE CARE

Our highly skilled team of comfort-care specialists shares a passion for providing physical, emotional and spiritual care.

Medical Director & Hospice Physicians

Work with your primary care physician to develop a customized care plan and oversee the delivery of all aspects of care. They ensure initial and ongoing hospice eligibility.

Nurse Practitioners

Work with the hospice medical director and hospice clinical team to ensure that you receive quality and safe care. Nurse practitioners also visit you to certify your continued hospice eligibility.

RN Case Managers

Specially trained Registered Nurses coordinate your personalized care, while also providing education and emotional support.

Physical, Occupational and Speech/Language Therapists

Assess your physical capabilities and needs to ensure individualized care.

CNA Hospice Aides

Certified nursing assistants help with personal care, bathing and meal assistance.

Medical Social Workers

Involved with you and your family to provide emotional support and reassurance as well as assist with navigating and obtaining needed services and resources.

Music Therapists

Support your emotional, cognitive and social well-being. After assessing your strengths and needs, music therapists provide treatment which can include singing, moving to, creating, and/or listening to music.

Massage Therapists

Reduce muscle tension and pain, and provide relaxation and stress relief.

Spiritual Counselors

Available to offer spiritual and emotional support to you and your family. Spiritual Counselors also work together with your own faith community leader.

Hospice Hound Teams

Bring a calming, friendly presence to your loved one. Each pet companion team includes a volunteer and their registered therapy animal.

Grief Specialists

Available to provide bereavement counseling and other supportive resources.

Volunteers

Offer comfort to patients in our care, including companionship, caregiver respite and presence during end-of-life services.

Eligibility for Hospice Care

People may qualify for hospice care when:

  • Health care goals change from cure to comfort
  • There is a diagnosis of life-limiting illness
  • Your physician supports the decision to choose hospice services

How to Initiate Hospice Services

As defined by Medicare, hospice care can begin with the diagnosis of an advanced illness with a limited life expectancy of six months or less.
