Job Description
1. PRINCIPAL ESPONSIBILITY (Essential) : To provide direct patient care to residents of Grand Village in accordance with our established nursing care procedures, and as may be directed by your supervisors. A. Essential Indicators a. Have current knowledge of assigned resident conditions at all times, b. Provide assistance in resident’s “activities of daily living”, i.e. eating, dressing, bathing, grooming, toileting, and other tasks as appropriate, c. Be aware of changes in resident’s condition and report same to the appropriate team leader, d. Provide emotional and physical comfort and safety for residents, e. Follow the resident care plan. B. Performance Indicators a. Residents will have all their physical needs met on a daily basis, b. The employee will immediately report changes in a resident’s condition to the appropriate team leader or supervisor, c. The resident care plan will be followed at all times 2. PRINCIPAL RESPONSIBILITY (Non-Essential) To accurately document data a. Accurate documentation using various methods of documentation. i.e. computer and paper documentation b. Concisely and accurately document care given, including nurse rehab. Performance Indicators a. All appropriate documentation of resident care/condition will be completed accurately and timely. 3. PRINCIPAL RESPONSIBILITY (Essential) to perform housekeeping chores in resident areas to provide a healthy, therapeutic, and safe environment for residents. a. To perform “ward order ” duties in resident rooms including beds. b. To monitor and maintain a safe environment for residents. Performance Indicators a. Resident rooms will be clean, orderly, and sanitary at all times. b. Any spills, messes, or unsanitary areas will be immediately cleaned up. c. All resident beds will be changed according to accepted nursing procedures and will be clean at all times. d. Resident over bed tables, bedside stands, closets, drawers, and other room furnishings will be kept clean, neat, odor-free, and in good repair according to Grand Village policies. 4. PRINCIPAL RESPONSIBILITY (Essential) To cooperate with policies and procedures so that continuity in serving nursing home needs will be ensured. a. To report for work and accept and complete assignments as scheduled b. To follow all nursing home policies and procedures. c. To follow all mandated state and federal regulations d. To follow legislative laws regarding resident rights and vulnerability. e. To maintain up-to-date knowledge of job responsibilities. f. To know and follow all facility policies on Infection control/Universal Precautions. Performance Indicators a. Report for and leaving the work area will be timely at all times. b. All variations in reporting for or leaving work procedure will be communicated to the designated supervisor. c. All nursing home policies and procedures will be followed. d. All work assignments will be completed as specified regarding. e. All work related injuries will be reported to the designated supervisor within 48 hours of the injury. f. To attend and successfully complete a minimum of at least 12 hours of pertinent in-service annually to include or in addition to completion of designated / assigned online training training during work time. g. All Infection Control/Universal Precaution policies will be followed at all times. h. To sign off on and date current position description annually. i. All work will be within acceptable state and federal guidelines. Nature and Scope Special Demands: The Nursing Assistant should have a genuine interest in geriatric nursing. Nursing Assistants work under direct supervision. Tact and courtesy are essential for dealing with residents, families, and visitors. Teamwork and good working relationships with fellow workers will continue to enhance a positive workplace attitude. Education: High School diploma or GED certificate. Licensure: Each Nursing Assistant must be registered and active by the State of Minnesota with the Minnesota Nursing Assistant Registry or eligible for registration. Experience: Experience in a long term care setting is helpful, although Grand Village is very willing to teach as you learn. Job Knowledge: Thorough knowledge of current accepted Nursing Assistant practices and skills are a must. Willingness to learn and to become familiar with the Grand Village organization, its functions, policies, regulations, and procedures and how these things relate to nursing service become essential for you as an employee. Must be able to satisfactorily perform the Functional Job Description and its Critical Demands as included with this position description. 2. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so the requirements of the Functional Job Description Critical Demands can be fully met. Employment variables: As Grand Village runs 24 hours per day - 365 days per year, employees are required to rotate shifts, work weekends, and holidays. Working Environment: Employees work inside well lighted and ventilated area. There is a potential for cuts and/or infections, strains or potential other injuries. We work with a variety of residents and a variety of personalities, so safety must always be foremost for yourself and other co-workers. The employee may be exposed to communicable diseases. You will receive a 2 step Mantoux (Tubeculin) testing upon your hire, and a Mantoux (Tuberculin) screening test annually. Freedom to Act: The Nursing Assistant works under direct supervision. Each Nursing Assistant works within guidelines as set forth by the Minnesota Department of Health within accepted standards, under guidelines of the Minnesota Department of Health, and within established duties to be completed.
Skills & Requirements
Minimum Qualifications: 1. Current Nursing Assistant Certification as registered with the Minnesota Nursing Assistant Registry 2. Registered with the Minnesota Board of Nursing or eligible for registration. Job Requirements: 1. Able to work rotating shifts, 2. Must be able to work on weekends, 3. Must be able to work holidays, 4.Must be able to provide direct resident care, 5. Must be able to lift,transfer,turn residents using proper technique 6. Able to accept assignments from team leaders, charge nurse, and DON 7 .Able to provide rehab orientated care 8.Must have a dependable work record 9.Must adhere to Nursing Department Uniform Policy 10.
