Job Description

The Nursing Assistant/Registered, provides compassionate care and service while assisting residents with bathing, dressing, grooming, eating, walking and toileting, under the direction of licensed nursing staff in accordance with the philosophy, policies and procedures of Emmanuel Nursing home, State and Federal rules and regulations. AA/EEO Task List: Resident Care __________◊ Know your residents, read Kardex (set of cares needed to meet each resident’s needs) and follow care plan. __________◊ Hand washing before and after each resident. __________◊ Answer call lights, bed alarms or chair alarms as quickly and take care of resident’s needs. When finished reset all alarms and place within resident’s reach. __________◊ Toilet residents per care plan or whenever they ask to go. Do perineal cares after every change. __________◊ Brush resident’s teeth, if false teeth put in resident’s mouth (am) if resident has own teeth, brush them or set them up to brush their own teeth. Remove false teeth (pm) and place in cleaner. __________◊ Turn on and insert hearing aids (am). Remove hearing aids, turn them off and store per care plan (pm). __________◊ Help residents pick out clean clothes and dress residents who need assistance and check on residents that generally dress self, see if they need help. __________◊ Comb resident’s hair whenever they get up. __________◊ Assist women with makeup and jewelry. __________◊ Remove women’s facial hair, shave men. __________◊ According to resident’s care plan, assist with range of motion (ROM) exercises. __________◊ Walk residents according to care plan. __________◊ Weigh residents and record weights monthly or more frequently per care plan. __________◊ Complete required charting for each resident. __________◊ Report any changes in resident’s condition, any new bruises or sores, to the nurse immediately. __________◊ Fill out the In and Out, Bowel Movement list, restorative program, Activities of Daily Living observations, toileting and turn schedule for each resident. __________◊ Report off to the nurse at the end of each shift. Cleaning __________◊ Make regular beds and wipe down frames and mattress for bath beds. __________◊ Clean men’s razors. __________◊ Clean resident’s glasses if they wear glasses and put on resident. __________◊ Clean utensils, bedpans and urinals weekly. __________◊ Clean tub following baths per cleaning procedure. __________◊ Clean wheelchairs and clean linen in wheelchairs. __________◊ Remove any soiled linen from beds or chairs and replace with clean linens. __________◊ Take out trash when straightening resident’s room. __________◊ Fill empty soap containers. Transportation __________◊ Get resident up for meals and activities make sure they are comfortable and safe in their chair. Put chair alarms on if resident has one. __________◊ Position call lights in resident’s room within reach. __________◊ Position side rails and restraints carefully. __________◊ Make sure residents are ready for all appointments, within and outside of the nursing home. Offer bathroom facilities prior to all appointments. __________◊ Report any equipment malfunctions to maintenance staff and fill out slip. Positioning/Transfers __________◊ Use gait belts for all transfers unless mechanical lift is used. __________◊ Reposition resident every 2 hours if they are unable to move self in bed or chair, make sure bed is in the low position and call light is within reach when you leave the room. __________◊ Nursing assistant must use teamwork, and be willing to help your fellow co-workers with Hoyer and PAL lifts if needed (know how to use mechanical lifts). Bathing __________◊ Complete partial bathing for residents. __________◊ Bath aides do whirlpool baths or showers- Residents get baths weekly or per their care plan. __________◊ Bath aides wash resident’s hair, also to help resident’s bathe- wash back etc. & help dress when bath or shower is completed. __________◊ Bath aides are responsible to trim fingernails and toenails of resident’s, unless diabetic. Nurses will trim nails of diabetic residents. Inform nurse when bath is complete and check to see if beauty shop has appointment available. Feeding/Hydration __________◊ Get residents to meals on time. If resident will be late, inform kitchen to hold tray until resident is ready. __________◊ Assist residents to eat as needed. __________◊ Residents need hydration, residents able to help themselves get a glass of water or juice close by. Residents that can’t help themselves are offered water or juice in-between meals and at night. Employment __________◊ Nursing Assistants need to be to work on time to receive report and follow dress code. __________◊ Complete pre-shift stretches. __________◊ Use PowerLift Techniques for transfers, repositioning, ambulation and bedwork.

Skills & Requirements

Position Requirements: 1. 16 years of age. 2. Ability to attend nursing assistant course and successfully complete tests to become a Nursing Assistant/Registered in the state of Minnesota, if not previously obtained. Employee must maintain Minnesota registration as a Nursing Assistant/Registered. 3. Completes TB/Mantoux requirements.
